Arena Sports Release and Waiver
Every Arena Sports participant must read, agree to, complete the form and sign the Arena Sports waiver in order to participate in activities at our facilities and satellite locations. All players under the age of 18 must have signature of Parent/Guardian.
Your privacy is important to us! The information you provide to us will only be used to communicate information about Arena Sports products and services such as game or class times, schedule updates, policy updates, etc.
PLEASE NOTE: If you fill out a paper waiver, you will also be required to complete an online, electronic waiver the first time you logon to www.arenasports.net. If you would like to fill out an electronic waiver instead of a paper waiver there are a few steps you'll need to take.
ELECTRONIC WAIVER INSTRUCTIONS FOR EXISTING PLAYERS/MEMBERS: If you've played at Arena Sports before, you probably have an online account with us. Just go to our member login and fill in your username and password. If you've forgotten your username or password, use our tool to get the logon information you need. Once you logon the electronic waiver will be the first thing you see. Just follow the instructions online.
ELECTRONIC WAIVER INSTRUCTIONS FOR NEW PLAYERS/MEMBERS: Visit our member login page and go to our New Customer Registration section. Enter in your email address and click signup. Then fill in your contact information. If you have any children, you can enter their information here too. Once the form is complete, click the Register button. A confirmation email will be sent to your email box. You'll need to go to your email box and open the email to confirm receipt of the email. Once you have clicked on the confirmation link, sign-in as a member on www.arenasports.net. When you first sign in, you'll see the electronic waiver to complete. From there, you can sign up for all kinds of fun!
